The AINP has introduced a bar code to some of our application forms to make it easier for those applying to the program. Download our Helpful hints (PDF) to assist you in filling out the application forms. You will need at least Adobe Reader 8.1 to fill out the application forms. If you do not have Adobe Reader 8.1, click on the icon below to download the program for free.
Click on the stream and/or category that applies to you to download the forms you must submit to the AINP.

Other AINP Forms
- AINP Change of Address and Contact Information Form
This form is to let AINP know that your address, email, or phone number has changed after you have applied.
- AINP Request to Add/Remove Family Member(s) Form
This form is to let AINP know that you have made a change to your family composition, after you have applied. Examples include the birth of a child, marriage or divorce.
- AINP Request for a Nomination Extension Form
This form is for individuals who have already been nominated by the AINP and who are requesting a nomination extension. Visit the After You are Nominated page for more information.
Employer-Driven Stream – Skilled Worker Category or International Graduate Category
If the Candidate and the Alberta Employer are eligible under the AINP Skilled Worker Category or the International Graduate Category, the Candidate and the Alberta Employer will need to:
- download the AINP Employer-Driven Stream Skilled Worker and International Graduate Category – Document Checklist (PDF) which outlines all forms and supporting documents required to apply to the AINP
- download Helpful hints (PDF) for filling out the application forms with a bar code
- have at least Adobe Reader 8.1 to fill out the application forms. Download Adobe Reader.
- complete all the required forms and include all required documents before mailing an application to the AINP. Your application will be returned to you if it is incomplete.
- submit all forms and supporting documents to the AINP together in one package. The Alberta Employer or the Candidate can mail the complete package. The AINP does not accept applications by fax.

As of March 14, 2011, all supervisors, foremen, or tradespersons working in an occupation designated as a Compulsory or Optional Trade in Alberta (including Chefs, Cooks, and Line Cooks) are no longer eligible to apply under the Employer-Driven Stream and must apply under the Strategic Recruitment Stream, Compulsory and Optional Trades Category.
ALBERTA EMPLOYER FORMS
The Alberta Employer must fill out and submit the following original AINP forms with original signatures:
- AINP 005: Employer-Driven Application for a Skilled Occupation
- If applying for a Manager/Supervisor in the foodservice industry include an Employer Compliance Declaration Form
- AINP 008A: Employer - Use of a Representative (if applicable)
CANDIDATE FORMS
The Candidate must fill out and submit the following forms:
- original AINP forms with original signatures
- copies of the following Citizenship and Immigration Canada (CIC) forms
The Candidate, their spouse/common-law partner, and any dependants over 18 (if applicable) must each complete and submit copies of the following CIC forms:
- IMM 5669 Schedule A: Background/Declaration
- IMM 5406 Additional Family Information
- IMM 5476 Use of a Representative (if applicable)
These are the CIC forms required to apply to the AINP’s Skilled Worker Category. Successful nominees will need to fill out additional CIC forms when applying for permanent residence. For help in filling out CIC forms, read the CIC Guide for Provincial Nominees.
Employer-Driven Stream – Semi-Skilled Worker Category
If the Candidate and the Alberta Employer are eligible under the AINP Semi-Skilled Worker Category, the Candidate and the Alberta Employer will need to:

The AINP recommends that all forms and supporting documents be sent to the AINP at the same time. However, Alberta Employers have the option of submitting the AINP 001: Employer-Driven Application – Semi-Skilled Worker Category application first along with the Employer's supporting documents. If they are approved for an allocation, all the other forms can be sent following that approval. However, this will increase the overall processing times for the application.
In the case of the Foodservice Industry (pilot project) it is required that all forms and supporting documents must be sent at the same time.
ALBERTA EMPLOYER FORMS
The Alberta Employer must fill out and submit the following original AINP forms with original signatures:
- AINP 001: Employer-Driven Application – Semi-Skilled Worker Category
- Employers must provide a Settlement and Retention Plan (PDF)
- AINP 005: Employer-Driven Application for a Semi-Skilled Occupation (PDF)
- When you open the AINP 005 form, ensure that you click on the box indicating “Semi-Skilled Worker”
- If applying under the Foodservice Industry (pilot project) include an Employer Compliance Declaration Form (PDF)
- AINP 008A: Employer - Use of a Representative (if applicable)
CANDIDATE FORMS
The Candidate must fill out and submit the following forms:
- original AINP forms with original signatures

Strategic Recruitment Stream – Compulsory and Optional Trades Category or Engineering Occupations Category
If the Candidate is eligible under the AINP Compulsory and Optional Trades Category or the Engineering Occupations Category, the Candidate will need to:

The Candidate, their spouse/common-law partner, and any dependants over 18 (if applicable) must each complete and submit copies of the following CIC forms:
- IMM 5669 Schedule A: Background/Declaration
- IMM 5406 Additional Family Information
- IMM 5476 Use of a Representative (if applicable)

Strategic Recruitment Stream – U.S. Visa Holder Category
The AINP is currently not accepting applications under the AINP U.S. Visa Holder Category. Applications postmarked after August 23, 2010 will be returned to you. Applications postmarked on or before August 23, 2010 that meet all program criteria will be accepted for processing and will be processed according to AINP Processing Times and U.S. Visa Holder Category criteria.
Family Stream
The AINP is currently not accepting applications under the AINP Family Stream. Applications postmarked after August 23, 2010 will be returned to the Alberta Relative. Applications postmarked on or before August 23, 2010 that meet all program criteria will be accepted for processing and will be processed according to AINP Processing Times and Family Stream criteria.
Self-Employed Farmer Stream
If the Candidate is eligible under the AINP Self-Employed Farmer Stream, the Candidate will need to:
- download the AINP Self Employed Farmer Stream – Document Checklist (PDF) which outlines all the forms and supporting documents required to apply
- complete all the required forms and include all required documents before mailing an application. Your application will be returned to you if it is incomplete.
- submit all forms and supporting documents to Alberta Agriculture and Rural Development together in one package. Alberta Agriculture and Rural Development will not accept applications by fax.
- mail your application to:
